Who is covered:
All players, coaches, managers, official scorekeepers, batboys, and volunteer workers on your registered team.
Covered Activities:
While participating in a regularly scheduled and supervised practice and play of Baseball. Also, coverage includes traveling directly to and from these described activities.
Benefits:
Accidents medical expense benefit (including dental) is paid in EXCESS of any expenses payable by other valid and collectible insurance to an Insured Person for an injury caused by a covered accident. Reimbursement is payable for reasonable and customary medical expenses, less the deductible of $50. Covered expenses must be incurred within 104 weeks of the date of the accident.
Maximum Benefit Amounts:
Players (16 and younger) $25,000
Coaches & Managers $10,000
Volunteer Workers, Batboys, & Team Scorekeepers $5,000
Claim Procedure:
When there is an injury, the Insured Person should get an itemized bill for the treatment. The Insured Person should then get a claim form from the League. The completed claim form, itemized bills, and explanation of benefits of other coverage is sent to:
